The HK4985‑registered aircraft lost contact around 09:23 on the morning of September 13 while en route from Condoto in the state of Chocó to Guaymaral Airport in Bogotá.

The signal from the aircraft's emergency locator transmitter was directed toward the mountainous area around Tatama Hill.

On September 14, a UH‑60 Black Hawk helicopter of the Colombian Air Force detected traces believed to belong to the aircraft near Tatama National Nature Park.

The Colombian Civil Aviation Authority (Aerocivil) later pinpointed the exact crash site in Pueblo Rico, Risaralda state, at an altitude of about 2,750 meters above sea level.

There was a medical team on board.

It was reported that the four medical staff on board returned to Bogotá after a free health service operation in the area. Perla Costanza Alvarez, Sandy Garcia, Maria Guzman and Yuliza Figueredo were listed as passengers; Mehmet Cankaya was identified as the pilot.

The medical team had provided services to about 600 people in Condoto as part of the health operation organized after the 7.4‑magnitude earthquake that struck the region on August 10.

More than 150 people took part in the search operations.

More than 150 people, including the Colombian Air Force, army, police, rescue teams, local residents and indigenous communities, joined the search and rescue efforts.

It was reported that the steep, mountainous terrain of the debris zone and heavy rain and fog made it difficult for teams to reach the area. The investigation into the cause of the accident has not yet been completed.

Officials said that after reaching the wreckage and examining the aircraft and evidence at the accident site, the cause of the accident could be determined.
