U.S. Tennessee Department of Corrections Commissioner Frank Strada announced that he will step down following the failed execution attempt of death row inmate Christa Pike. Tennessee Governor Bill Lee said Strada will leave his post this month.

Strada said his resignation was the right decision for the people of Tennessee. Strada, who called for an independent investigation into the failed execution, said he will work to bring all aspects of what happened to light.

Execution Failed Twice

A 50‑year‑old Christa Pike was scheduled to be executed on Wednesday, September 30, by a lethal injection of the drug pentobarbital. However, despite the sedative pentobarbital being administered twice, the execution could not be completed; Pike was subsequently taken to a hospital.

Pike was connected to a breathing apparatus.

Pike’s attorneys, in a statement made on Friday, said the inmate’s consciousness had been closed off, he had been sedated, and he was attached to a breathing device. According to the lawyers, Pike suffered severe injuries on both arms, and the medical team at the hospital is attempting to clear the pentobarbital from his body.

The maximum of seven needles was used

According to documents submitted to the court by Pike’s legal team, the execution team used at least seven needles to open the vein pathway. Lawyers argued that the vein routes were not properly positioned or that the veins had been damaged, and that a second dose of pentobarbital was administered despite this.

Executions halted in Tennessee

Attorney General Bill Lee halted the remaining executions scheduled for the rest of 2026 in Tennessee after Pike’s failed execution. The state administration is conducting an independent inquiry to determine how the incident occurred.

Strada maintained that it acted in accordance with the protocol established by the Tennessee Department of Correction.

Nevertheless, during the ongoing independent inquiry, Strada argued that its removal from office would be in the interest of the people of Tennessee.

Why is Pike's Execution in the News?

Pike was sentenced to death in 1995 for murdering his 19‑year‑old former classmate Colleen Slemmer. At the time of the crime, Pike was 18, and if he had been executed he would have become the first woman to be executed in Tennessee since the 1800s. The U.S. Supreme Court issued the decision that paved the way for the execution on September 30.

This Year’s Second Failed Attempt

Pike’s execution attempt became the second failed execution in Tennessee this year. In May, the execution of Tony Carruthers was halted because officials could not establish one of the necessary vascular access routes. After the Pike incident, the Tennessee administration began reviewing execution practices again.

Commissioner Role Began in 2023

Strada was appointed head of the Tennessee Department of Corrections in January 2023. During his tenure at the Arizona Department of Corrections, Strada had also encountered problems with establishing vascular access in some executions in Arizona.
